Primary Health Care Inc. seeks a mission-driven Faculty Physician to provide high-quality primary care while teaching and mentoring clinicians in a community health center setting. Based in Des Moines, you will deliver comprehensive, patient-centered care to diverse and underserved populations, supervise and precept learners, and support evidence-based, team-based practice. The role emphasizes collaboration, continuous learning, and quality improvement in a respectful, inclusive culture committed to health equity and community impact.
Provide comprehensive primary care to a diverse, largely underserved patient population
Precept, teach, and mentor residents, students, and advanced practice clinicians
Collaborate with an interdisciplinary care team to deliver integrated services
Apply evidence-based guidelines and quality improvement practices
Document care accurately in the electronic health record (EHR)
Participate in clinical teaching conferences and educational activities
Contribute to care coordination, population health, and chronic disease management
Engage in activities that advance PHC’s mission and health equity goals
Primary care medicine (adult and/or family practice)
Chronic disease management (e.g., diabetes, hypertension)
Outpatient clinical procedures (appropriate to specialty)
Clinical teaching and precepting
Electronic health record (EHR) documentation
Evidence-based practice and guidelines
Care coordination and team-based care
Population health and panel management
Quality improvement methods
Culturally competent communication
